Very similar is Okenia kondoi. They can be distinguished by the number of dorsal appendages: Okenia kondoi has four pairs, Okenia nakamotoensis has five pairs.
It was described by Hamatani as Hopkinsia nakamotoensis, Gosliner (2004) proposed synonymising the genera Hopkinsia, Sakishimaia, and Hopkinsiella with Okenia.
